The aim of the present research was to evaluate smoking prevalence and peculiarities of chronic obstructive pulmonary disease (COPD) among native inhabitants of the North. We examined 433 (168 male and 265 female) native rural inhabitants of Taymir and Evenkia districts at ages from 28 to 70. We used standard epidemiological methods: questionnaire in diagnostics of chronic lung diseases; anthropometry; examination by pulmonologist; X-ray studies of the lung; measurement of lung function. The stage of obstruction and the stage of COPD were estimated according to GOLD standards. We found that 53% of the examined were smokers. Among the smokers there were 56.2% male and 43.8% female. The prevalence of COPD was 21.9%. At the same time 53.5% of COPD patients were smokers. The most numerous groups of COPD patients were female aged 40 to 49 and male aged 50 to 59. COPD course in the extreme conditions of the North is characterised by rapid progress of obstructive and restrictive disturbances of external respiration. Hyperventilation and restrictive changes were revealed more often in female smokers. The study suggested that adverse smoking effects on pulmonary function were greater in women than in men. So, smoking and extreme conditions of the North provoke high prevalence and heavier course of COPD in the native inhabitants of the North.
