TUBERCULOSIS IN NORTH SBERIAN AND FAR EAST INHABITANTS

For the last 10-15 years the epidemic situation with tuberculosis (TB) became quite acute both.in the Russia, and in the world. Novosibirsk TB Research Institute is a center of pursuance and coordination of scientific investigations and providing of antituberculosis cure in Siberian and Far East federal regions. These territories are represented by North of Asia and have pronounced climatic, geographical, demographic and social-economical peculiarities. General TB morbidity rates in the Northern territories vary from 60 (per 100 thousand inhabitants) in Chukotka autonomous region to 350 in Koryak autonomous region. The indigenous population morbidity rate is 3-4 times higher than among non-native inhabitants. Increase of morbidity rate among children and adolescents indicates very tense epidemic situation too.

International experience of phthisiology of the last decades has shown that separately provided TB control activities on social recovery are not effective enough, in particular, antibacterial therapy, because of the multiple-factor of that problem. Novosibirsk TB Research Institute in cooperation with Research Institutes of Siberian Academy of Sciences and Scientific Center “Vektor” provides actions on improving of system of support to TB control facilities of the Northern territories on following directions:

- organizational activities on interaction with authorities at all levels and general health care system to improve detection, prevention of TB and health care service of indigenous and different groups of non-native inhabitants;

- prediction of clinical course and treatment outcome based on study of the clinical, immunogenetic, morphological peculiarities of pulmonary and extra-pulmonary TB in aboriginal and non-native inhabitants;

- study of bacteriological and immunogenetic peculiarities of Mycobacterium tuberculosis to improve TB diagnostics, to use TB drugs in accordance to the data on drug-resistance and to provide timely correction of treatment course;

- development and use of the methods of target delivery of TB drugs based on study of drug metabolism and their side effects;

- development and implementation of individual treatment of TB patients according to their mental and physiological peculiarities and co-diseases, that can be regional pathology for indigenous population and consequence of disadaptation for newcomers;

- providing of preventive activities taking into account of the peculiarities of life style, traditional culture of the North peoples, their production activity, and features of acclimatization of the human to the high latitudes.

Investigations of the TB peculiarities in northerners are of theoretical interest, and of practical importance for improving of TB control methods in Circumpolar territories.
