Arterial intima-media thickness (IMT) is a sensitive marker of early atherosclerotic changes, which is considered in relation to cardiovascular prognosis.
METHODS: The representative population sample (491 men, aged 18-60) was examined by high-resolution carotid ultrasonography to investigate the patterns, determinants and prognostic significance of carotid artery atherosclerotic disease (CAAD) in Siberian city population (Novosibirsk).
RESULTS: The prevalence of CAAD in population by presence of plaques and/or by intima-media thickening (IMT>0.9 mm) comprised 17.8%. The average IMT was of 0.65 mm (0.01) in entire sample, and consequently increased by age. Conventional factors (age, systolic blood pressure, smoking and body mass index) explained the variability of IMT by 21%. The hypertension, high cholesterol level, low physical activity and low education level were the independent determinants of CAAD. Over the 10-year follow-up period the CAAD increased the age-adjusted risk of myocardial infarction by more than three times (RR=3.4; 95%CI: 1.0-11.8; p=0.039) and the risk of death from all causes by nearly six times (RR=6.3; 95%CI: 2.9-14.0; p<0.001).
CONCLUSION: We found substantial prevalence of carotid atherosclerosis in Siberian male population. The arterial wall lesion strongly predicts the risk of cardiovascular outcomes and mortality.
