PSYCHOPHYSIOLOGICAL APPROACHES TO THERAPY OF FIRST EPISODE PULMONARY TUBERCULOSIS PATIENTS

There is strengthening global trend to treat a patient instead of a disease. assuming evidenced psychosomatic relationships, i.e. a link between psychoemotional status of the patient and disease. This is a rule for infectious diseases, including tuberculosis (TB). It is well known that in era of chemotherapy treatment of TB patients the main emphasize majority is placed on pharmacological approaches. Thus personal and individual psychoemotional factors are taken into account in less extent while developing treatment and rehabilitation strategies. Meanwhile, specific antibacterial therapy could not guarantee complete rehabilitation of patients with TB. So, appliance of methods of diagnostic and monitoring of the condition of psychoemotional sphere of the patient, characteristics of his neurovegetative status and technologies of correction of the detected abnormalities of individual psychoemotional area in TB control practice is extremely important. Associations between psychoemotional status the patient with TB and clinical time course have been found in several cases. In such a way, the purpose of that research is comparative assessment of dynamics of individual features of psychophisiological, neurohormonal and neuroimmune status of patients with TB receiving standard pharmacological treatment vs. patients with TB receiving standard treatment combined with psychophisiological method of antistress therapy. We have studied newly detected patients with infiltrative pulmonary TB at the age of 20-45 years. According to the treatment scheme, patients were divided into two groups: group of standard antituberculosis therapy (STT) and group receiving neurotechnological correction (NTC) in addition by the method of audio-visual- vibrotactile stimulation (2-3 times a week for 30 minutes). This method is based on synchronous presentation of visual, acoustic and vibrotactile stimuli in the range of brain bioelectrical activity of 4.5-16 Hz. Appliance of that method leads to symmetrization of the balance between branches of sympathetic and parasympathetic systems, psychic and somatic stress relief. As a result it was determined that at the beginning of the research patients of both control groups had more or less similar psychic, psychomotor, neurovegetative and neuroimmune indexes, but in 30 days of combined therapy in NTC group there was proved optimization of the vegetative balance (strengthening of vagotonic and reduction of sympathetic influence on regulation of heart rate) against a background of lower rates of cortisol and ACTH of plasma. Also there was mentioned favorable dynamic of specific clinical presentations of the disease. The findings evidence efficiency of the new approach in phthisiatric practice.
