Environmental health

Surgical treatment of malignant tumors of liver, ductus choledochus and pancreas in endemic nidus of opisthorchiasis.

Objective: The purpose of this study is to learn peculiarities and to improve the results of treatment of malignant tumors of liver, ductus choledochus and pancreas in the nidus of opisthorchiasis.

Methods: The work is based on observation of 66 patients with malignant tumors of hepatobiliary zone, whom are produced 3 category of operations: extensive liver resections (32-48,5%), hepatococholedochal resections (20-30,3%) and pankreato-duodenal resections (14-21,2%).

Research group in category I – 17 patients (72,4%) were operated with using high technologies: ultra sonic destructor – aspirator Cusu-Exel, argon – plasmic coagulator Erbe-PC-300, clipator Auto-Sonix, Liga Shu, Tacho-komb plates and other. In comparison group by 15 patients (28,6%) were produced anatomical liver resections without using high technologies.

Category II: Hepatocholedochal resection: by 9 patients (45%) high technologies were used. By 11 patients (55%) were produced operations without using high technologies.

Category III: Pankreato-duodenal resections were executed. By 10 patients (72,4%) – reconstruction phase was executed – with forming all anastomoses on the isolated intestinal loops. By 4 patients (28,6%) was produced PDR with forming all anastomoses on a certain intestinal loop.

Results: In Khanti-Mansiisk autonomius region – Ugra – cholangiocellular cancer (88,2%) is predominate. Tumors on the background of opisthorchiasis showed by expressing of proliferation process, which make conditional on big sizes of the primary nidus, inclination to necrosis and disintegration. High technologies, which used in the liver resections in the research group, allowed statistically reliable reduce the frequency development of post-operations complications, essential (from 26,7% to 5,7%) reduce lethality. As for the hepatocholedochal resections – there were no complications. As for PDR in research group there were 2 complications, in comparison group there were 3 complications.
