CRITERIUM OF FORMING OCCUPATIONAL PATHOLOGIES IN THE WORKERS

Concentrating the big industrial complexes at the territory of Eastern-Siberia leads to the growth of morbidity rate among the working population. The reduction in natural organism responsness is known to be one of the causes of the development of occupational and occupation-stipulated diseases. Immunity system is the first to response to unfavourable production factors. With this connection, the studies on revealing the common regularities and the features in immune response disorders of the worker organisms exposed to different production factors have been performed. We have performed a clinical – immunological examination of the persons working at the enterprises of Oil-Chemical, Chemical, Pulp and Paper, Microbiological industries as well as the persons of vibration-dangerous occupations, medical workers, the fire fighters. The study results allowed to characterize the peculiarities of immune deficient states induced with the exposure to different production factors which are differed in regulatory (T-lymphocyte subpopulation activity, cytokine production) and the effector functions of immune system. Thereto, the role of organism immune response disturbances has been shown in forming the pathologies of principal organism organs and systems. The increased role of organism hypersensitivity presence has been revealed in the workers to a number of production allergens (products of a microbiological synthesis, epichlorhydrine, methyl sulfuric compounds, phenol and so on). It should be noted that the occupational allergic diseases were offer revealed among the medical workers (53,5%), then at the plants of mechanical engineering industry (12,2%), chemical industry (12,2%), building industry (4,8%), pharmaceutical (4,8%), metallurgical (2,4%) and at the objects domestic services (4,8%). In some cases autoimmune response development may play an important role in the development mechanisms of occupational pathology occurrence. The high levels of autoantibodies to the antigens of the nervous tissue (93,7% of the cases) and to the denaturired DNA (81,03% of the cases) in the patients with toxical encephalopathies as well as autosensibilization development to antigens of cartilage tissue in 58% of the patients examined with vibration-induced diseases were found to testify above-stated. Forming and organism response outputs to pathogenic production factors were found to depend on their exposure conditions to the workers (duration, dose load, genetic predisposition). It should be noted that the exposure to production factors inevitably induces a number of compensatory processes which may restore the alterated immune system functions under favourable conditions (stopping exposures to production harmful factors). The examination results of the workers during their working at the Pulp and Paper Combinate in the of Baikalsk as well as -2 years after stopping exposures to unfavourable production factors were found to testify above-stated.

The findings testify the pronounced immune homeostasis alterations among the workers with the work length which may play a definite role in the development pathogenesis of occupational pathology. The general regularities and disturbance features revealed in the immune system, which were induced with the exposure to physical and chemical production factors, became the for grounding a complex of treating and preventing measures. Taking into account that the development and clinical manifestation of occupational pathology may occur later than the alterations in immune homeostasis would be registered, the formation of continuous health state monitoring among the workers with aim of revealing the prognostically unfavourable signs may be of great practical importance.
