THE METABOLIC SYNDROME DIAGNOSTIC CRITERIA FREQUENCY OF OCCURRENCE IN THE THERAPEUTIC PATIENTS

The metabolic syndrome (ÌS) uniting such diseases as type 2 diabetes, obesity, arterial hypertensia (ÀH), ischemic heart disease, system atherosclerosis of vessels, can be reasonably attributed to so-called «illnesses of a civilization». Tissue insulin resistance (IR) is the central part of the given syndrome pathogenesis and, accordingly, of all its components which have been mentioned above. Summarizing researches of last years, it is possible to name most important of MS diagnostic criteria: hyperinsulinemia and IR; overweight; imparments of a carbohydrate metabolism (ICM): type 2 diabetes and glucose tolerance disturbance; dyslipidemia (DLP); the cardiovascular pathology caused by the system atherosclerotic process; hyperuricemia, gout and urolithic diseases associated with it; a polycystic-ovary syndrome. In 2001 the ÌS diagnostic criteria have been specified by experts of the USA Educational program on cholesterol. The main establishing criteria were marked out and more precisely formulated from the full list of possible criteria; three of them actually allow to diagnose ÌS: visceral obesity, hypertriglyceridemia (HTG), decrease in a level of high-density lipoprotein cholesterol, ÀH (an arterial pressure of >130/85 mmHg), ICM.

The comparative analysis of various ÌS diagnostic criteria frequency of occurrence in SCCEM therapeutic clinic patients constantly living in Western Siberia and Sakha Republic (Yakutia) was the purpose of the research. Work is executed on the case records archival material of the patients hospitalized in clinic in 2005.

As a result of the retrospective analysis of 412 case records (158 person constantly lived in Sakha Republic (Yakutia) at the moment of entering the clinic, 254 persons - in Western Siberia), selected for the analysis by a casual sample method, it was found, that in all samples with the greatest frequency met DLP (75 %); further the overweight (69 %), a cardiovascular pathology (59 %), hyperuricemia (19 %), ICM (19 %), in decreasing order, followed.

While comparing these parameters in two allocated groups of patients it is revealed that DLP (77 % and 74 % accordingly), diseases of cardiovascular system (68 % and 54 %), associated with an atherosclerosis of vessels, and also purines metabolism infringements (25 % and 15 %) is met authentically more often in comparison with inhabitants of Western Siberia among patients constantly living in Sakha Republic (Yakutia),. On the contrary, ICM (13 % and 22 %) and overweight (65 % and 72 %) were found out in the patients living in Western Siberia, though distinctions only for ICM are authentic, while for overweight they have character of the tendency more often.

Thus, ÌS metabolic criteria in the patients living in Western Siberia, occur rather more often, than in patients from Sakha Republic (Yakutia), pathogenenetically connected with insulin resistens being, on the one hand, her direct consequence, and on the other hand, promoting her progressing. ÌS clinical criteria that are caused, probably, by more rigid climato-ecological conditions of this region, which chronic effect on an organism conducts to the expressed tension and a fast exhaustion of an organism adaptive system reserves and, as consequence, to earlier development and adverse current of clinically expressed pathology prevail in patients from Sakha Republic (Yakutia).
